How Our Care Model Works
This is the care model we have designed to support everyone involved in a child’s care journey, including the patient, family, nurses, physicians, and support staff. When each part of the care team works together, the child at the center of that care has the greatest opportunity to thrive at home.
RN-Led Oversight
Every case is overseen by a supervising RN who manages care both inside and outside of the home. We place a strong emphasis on RN staffing whenever possible, while LPNs remain closely supervised through ongoing assessments, supervisory visits, and clinical oversight.
Collaborative Care Coordination
We maintain ongoing communication with physicians, specialists, and families throughout the patient’s care. Our team provides clinical updates, coordinates physician plan renewals, and works closely with families who play an important role in their child’s daily care and long-term progress.
Continuity & Support Systems
Our nurses are supported through ongoing training, digital documentation systems, supervisory oversight, and staffing coordination. We also work to carefully match nurses with families to help create stronger long-term relationships, consistency, and stability within the home.
